RT3 fits into the T3 cell receptor sites but doesn't activate them, like a key that fits but won't turn the lock.

That's why many people who take thyroid medication may have "normal" labs on paper but still feel exhausted, foggy, cold, low temperature, and struggle with weight. High RT3 is often the missing explanation.

Why Thyroid Medication Can Lead to High RT3

Most thyroid medications, especially T4-only options like Levothyroxine or Synthroid, are designed to replace what the thyroid isn't making. That sounds logical—but these meds don't help your body convert T4 into T3. They simply add more T4.

If conversion is already impaired, this creates a surplus of T4. The body, trying to protect itself, redirects that hormone into Reverse T3. So instead of increasing energy or improving symptoms, the medication may actually worsen the imbalance. RT3 builds up, blocks the active hormone, and leaves you stuck in a hypothyroid state—even though your numbers look fine.

Why T3 Thyroid Medication Isn't the Magic Bullet Either

I know because that was my thought process. I kept thinking I was "one medication change away from getting better." And in some cases, T3-only or combination therapy can provide some symptom relief in some situations. But here's the catch: forcing active thyroid hormone into the body still doesn't solve the underlying Thyroid Deficit.

T3 is a potent and stimulating hormone. It metabolizes in the body very quickly — within 4 to 6 hours — which means it can rapidly increase blood pressure, pulse, and heart rate. That spike triggers adrenal stress and high cortisol, which can cause anxiety, irritability, headaches, heart palpitations, hair loss, and — you guessed it — weight gain.

Adrenal stress can cause weight gain, edema, and swelling — particularly around the midsection. So in people with an already-stressed system, T3 can make the weight problem worse, not better.

What makes T3 especially complicated is that once you're taking it, standard thyroid lab testing becomes almost impossible to use as a guide. Typically, Free T3 will be at the very top of the range, Free T4 will be low, TSH may be suppressed, and Reverse T3 will be below 12. Some online thyroid communities see these lab ranges as a "win" and stop there.
