If you don't have a thyroid, it's common to hear you don't need iodine. Social media and even doctors are quick to perpetuate this myth because they simply don't understand the importance of iodine. It sounds logical — iodine is "for the thyroid," and you no longer have one. But that myth prevents people from supporting the rest of their body, which still runs on iodine. As a result, many people continue to suffer from symptoms including fatigue, brain fog, and slow metabolism — even on their medication.

The short answer: yes, you still need iodine. Here is why.

The myth: "No thyroid means you don't need iodine."

The truth: The thyroid is one gland that uses iodine, but it's not the only one. Iodine is needed by glands and tissues throughout the body — the breast, ovaries, uterus, prostate, gallbladder, brain, stomach, lacrimal (tear) glands, and more. Without enough iodine, these tissues can run dry and become unhealthy.

What the thyroid does with iodine — and what changes without it

Your thyroid is a small gland at the base of your neck. It uses iodine as the raw material to build its two hormones — T4 (thyroxine) and T3 (triiodothyronine) — which regulate metabolism, energy, mood, temperature, and more.

When that gland is gone — from surgery (thyroidectomy), radioactive iodine treatment, autoimmune disease, or being born without one — it can no longer make those hormones. That is why doctors prescribe thyroid hormone replacement, usually a T4 medication such as levothyroxine or Synthroid.

But here is what surprises people: even on medication, many still deal with fatigue, brain fog, weight gain, and cold sensitivity. Often, medication alone is not enough without the minerals your body needs to use it — and much of your body still runs on iodine. Two minerals still matter without a thyroid: iodine and selenium.

Your body still has to convert your medication

Even without a thyroid, there is a step your body cannot skip.

T4 is the inactive, storage form of thyroid hormone. T3 is the active form your cells actually use. Whether T4 comes from a thyroid or from a pill like Synthroid, your body still has to convert it into T3 before you feel the benefit. That conversion happens outside the thyroid — in the liver, kidneys, gut, and brain — through enzymes called deiodinases. (The process is literally de-iodination: removing an iodine atom from T4 to make T3.)

There are three deiodinase types: D1 (liver and kidney), D2 (brain, pituitary, brown fat), and D3, which inactivates hormone by producing Reverse T3 and rises during stress or illness.

Here is the part that matters most without a thyroid: deiodinase enzymes are selenium-dependent. If you are low in selenium, conversion slows — plenty of inactive T4, too little active T3, and hypothyroid symptoms even on medication. Iodine is used far beyond the thyroid

Iodine comes in two forms, and different tissues use them differently:

  • Potassium iodide — the form the thyroid concentrates through its iodide pump (the sodium-iodide symporter, or NIS), and the form the skin uses.
  • Molecular iodine — the form used heavily by hormone-sensitive tissues, particularly the estrogen-sensitive ones: the breast, ovaries, and uterus. [1]

The same iodide pump that feeds the thyroid is also present in other tissues — the salivary glands, the stomach lining, the prostate, the lactating breast, the skin. [2] Even without a thyroid, these tissues still take up and use iodine. That is why the answer is absolutely: a thyroidectomy changes what your thyroid can do, not what the rest of your body still requires.

The ovaries can also make thyroid hormone

There is one more reason iodine matters for women without a thyroid. According to Dr. Jorge Flechas — a physician known for his clinical work on iodine — the ovaries are the one gland outside the thyroid that can produce thyroid hormone, and that production depends on iodine. [4] By his account, the thyroid uses the most iodine of any gland, and the ovaries are next in line. So even after a thyroidectomy, supplying iodine may support the small amount of thyroid hormone the ovaries can contribute.

Iodine is not a replacement for your medication

Be clear on this. If your thyroid has been removed or cannot function, your body can no longer produce its own hormone, and no amount of iodine or selenium restores that. You need your prescribed replacement; iodine and selenium don't replace it.

Where that iodine should come from

Most iodine supplements built "for the thyroid" contain only potassium iodide — the thyroid's form.
