What Is Thyroid Conversion?
You rely on your thyroid gland to make hormones that set your metabolic rate and keep your whole system running. But making hormone is only the first step. There's a second, vital step where things can go wrong — and many thyroid patients have never heard of it.
Your body makes T4 thyroid hormone — or you take a medication with T4, like Levothyroxine. But T4 is largely a storage form. The next step is when your body converts that T4 into T3, the active hormone your cells actually use for metabolism, energy, focus, and mood. That conversion step is the work of enzymes called deiodinases, and it depends on having the right raw materials on hand. [1]
When conversion works well, life can look pretty good. But some people don't convert T4 into T3 the way they should. Instead, their body shunts more of that T4 into Reverse T3 — a near-mirror-image molecule that does the opposite of what active T3 does.

Is There a Lab Test for Conversion and Reverse T3?

What Causes Reverse T3?
Here's where it gets interesting. Whether you have a thyroid, Hashimoto's, thyroid nodules, or no thyroid at all, conversion still has to happen — and it can still get blocked.
If you've been diagnosed with low thyroid function, your doctor will most likely prescribe a medication, often one containing T4 or NDT. But several everyday factors can slow the conversion of T4 into T3, even when you aren't sick — and some of them may surprise you.
7 Stressors that Affect Thyroid Conversion and Metabolism
❌ Calorie-restricted diets. Because thyroid patients so often struggle with weight, severe calorie cutting is common — and it backfires. Cut too low and your body reads it as scarcity and slows your metabolism, raising Reverse T3. When you eat normally again, the weight can come back fast. This is also why the lab picture gets so confusing. If your doctor only checks TSH and T4, more medication can look fine on paper, because the panel never shows the Reverse T3. A surprisingly common variation in that gene — present in an estimated 15% of people, and more in some groups [5] — makes that enzyme work less efficiently, so your cells can come up short on active T3 even when your bloodwork looks perfectly normal. This is the piece that should change how you think about medication. Standard thyroid medication like Levothyroxine is pure T4 — it gives your body more raw hormone and counts on your body to convert it. But if conversion is exactly where you're weak, more T4 doesn't fix the bottleneck. Research on people carrying this variation found their active T3 stayed low on T4-only medication even when their TSH looked normal and "treated."
